Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis with collection of fluid in the abdominal cavity (ascites)
- Swelling due to excess fluid retention (edema)
- Hyperaldosteronism
- Resistant edema associated with secondary hyperaldosteronism
Pharmacology
- Spironolactone and furosemide combination
- Complementary mechanisms and sites of action
- Additive or synergistic diuretic effects
- Inhibition of Na+/K+/2Cl- co-transporter in the ascending Loop of Henle by furosemide
- Antagonism of aldosterone action by spironolactone
- Potassium-sparing diuretic and loop diuretic
Dosage & Administration
- Furosemide 20 and spironolactone 50 mg
- Daily dosage range of 20 to 80 mg of furosemide
- Daily dosage range of 50 to 200 mg of spironolactone
- Furosemide 40 and spironolactone 50 mg
- Dosage for stabilized patients requiring a higher dosage
- Not suitable for use in children
Interaction
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Increased levels of cardiac glycosides such as digoxin in the blood by spironolactone
- Hypokalemia risk with corticosteroids
- Reduced or abolished diuretic effects when used with indomethacin and other NSAIDs
- Increased ototoxicity of aminoglycoside antibiotics by furosemide
- Reduction of natriuretic and anti-hypertensive effects by sucralfate when used with furosemide
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function (creatinine clearance <30 ml/min)
- Hyperkalemia
- Addison's disease
- Hypersensitivity to spironolactone, furosemide, or sulphonamides
Side Effects
- Headache and drowsiness by spironolactone
- Gastrointestinal distress, including cramp and diarrhea
- Ataxia and mental confusion
- Gynaecomastia and breast enlargement
- Endocrine disorders including hirsutism and impotence
- Hyponatremia and hyperkalemia
- Dehydration and reduction in blood volume leading to circulatory collapse
- Serious depletion of potassium and magnesium leading to cardiac arrhythmias
Pregnancy & Lactation
- Crosses placental barrier in pregnancy
- Potential fetal abnormalities with furosemide use
- Excretion in breast milk during lactation
Precautions & Warnings
- Caution in patients liable to electrolyte deficiency
- Caution in diabetes, enlarged prostate, hypotension, and hypovolemia
