Medicine Details

Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with fluid collection in the abdominal cavity (ascites)
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Pharmacology

  • Spironolactone and Furosemide have complementary mechanisms and sites of action
  • Additive or synergistic diuretic effect when given together
  • Furosemide inhibits the Na+/K+/2Cl- co-transporter in the ascending Loop of Henle
  • Blocks reabsorption of sodium, potassium, and chloride ions
  • Spironolactone inhibits the reabsorption of sodium exchange for potassium at the distal tubule

Dosage & Administration

  • Furosemide 20mg and spironolactone 50mg: 1-4 tablets daily
  • Furosemide 40mg and spironolactone 50mg: 1-2 tablets daily
  • Dosage may vary according to patient's response

Interaction

  • Increased risk of hyperkalemia when taken with ACE inhibitors or potassium salts
  • Increases levels of cardiac glycosides in the blood when taken with digoxin
  • Possible reduction in blood pressure lowering and diuretic effects when used with indomethacin or other NSAIDs

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function (creatinine clearance <30 ml/min)
  • Hyperkalaemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Side Effects

  • Headache, drowsiness, gastrointestinal distress, cramp, diarrhea
  • Ataxia, mental confusion, skin rashes
  • Gynaecomastia, breast enlargement, hirsutism, voice deepening, menstrual irregularities, impotence
  • Transient increase in blood-urea-nitrogen concentrations, mild acidosis
  • Hyponatremia, hyperkalemia, dehydration, reduction in blood volume, circulatory collapse, vascular thrombosis, embolism
  • Serious depletion of potassium and magnesium, cardiac arrhythmias

Pregnancy & Lactation

  • Spironolactone and its metabolites may cross the placental barrier
  • Furosemide may cause fetal abnormalities
  • Need for risk-benefit assessment in pregnant women
  • Metabolites of Spironolactone detected in breast milk
  • Furosemide excreted in breast milk

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, hypovolemia

Therapeutic Class
