Pharmacology
Rabeprazole suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell.
Dosage
- 20mg once daily for active duodenal ulcer and active benign gastric ulcer
- 20mg once daily for 4-8 weeks for GERD
- 10mg or 20mg once daily for GERD maintenance
- 60mg to 120mg once daily for Zollinger-Ellison Syndrome
Administration
Should be taken in the morning before eating, should not be chewed or crushed, but swallowed whole.
Interaction
Potential interaction with ketoconazole, itraconazole, and atazanavir. No interaction with liquid antacids observed.
Contraindications
- Hypersensitivity to the active substance or excipients
- Contra-indicated in pregnancy and during breastfeeding.

Precautions & Warnings
- Malignancy should be excluded prior to commencing treatment
- Long-term treatment may increase risk of hip, wrist and spine fracture
- Hypomagnesaemia
- Interference with laboratory tests
- Influence on vitamin B12 absorption
- Subacute cutaneous lupus erythematosus (SCLE)
- Patients with hepatic impairment need caution
- Co-administration with atazanavir not recommended
- Possible increased risk of gastrointestinal infections
Use in Special Populations
- No dosage adjustment necessary for renal or hepatic impairment
- Not recommended for use in children
Overdose Effects
Effects generally minimal, reversible without further medical intervention. No specific antidote known. Not dialysable.
