Medicine Details

Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with ascites
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Pharmacology

  • Potassium-sparing diuretic
  • Loop diuretic
  • Complementary mechanisms and sites of action
  • Additive or synergistic diuretic effect
  • Inhibits Na+/K+/2Cl- co-transporter
  • Blocks reabsorption of sodium, potassium, and chloride ions
  • Induces potassium loss
  • Inhibits reabsorption of sodium in exchange for potassium at the distal tubule
  • Antagonizes the action of aldosterone

Dosage & Administration

  • Furosemide 20 and spironolactone 50 mg: 1 to 4 tablets daily
  • Furosemide 40 and spironolactone 50 mg: One to two tablets daily
  • Use in children not recommended
  • Elderly patients may excrete the drugs more slowly

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Elevated levels of cardiac glycosides such as digoxin in the blood
  • Reduced or abolished blood pressure lowering and diuretic effects when used with indomethacin
  • Increased ototoxicity of aminoglycoside antibiotics when used with Furosemide
  • Reduced natriuretic and anti-hypertensive effect of Furosemide when used with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function (creatinine clearance <30 ml/min)
  • Hyperkalemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Side Effects

  • Headache and drowsiness
  • Gastrointestinal distress including cramp and diarrhea
  • Ataxia, mental confusion, and skin rashes
  • Gynaecomastia
  • Endocrine disorders including hirsutism, deepening of voice, menstrual irregularities, and impotence
  • Transient increase in blood-urea-nitrogen concentrations
  • Mild acidosis
  • Hyponatremia and hyperkalemia
  • Excessive diuresis leading to dehydration and reduction in blood volume
  • Serious depletion of potassium and magnesium leading to cardiac arrhythmias

Pregnancy & Lactation

  • Spironolactone and its metabolites may cross the placental barrier
  • Furosemide may cause fetal abnormalities
  • Use of Furosemide only in women when appropriate contraceptive measures are taken or if benefits justify the potential risks to the fetus
  • Metabolites of Spironolactone detected in breast milk
  • Discontinue breastfeeding if Furosemide treatment is essential

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, and hypovolemia
