Medicine Details

Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with collection of fluid in the abdominal cavity (ascites)
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Pharmacology

  • Complementary mechanisms of Spironolactone and Furosemide
  • Additive or synergistic diuretic effect
  • Inhibition of Na+/K+/2Cl- co-transporter by Furosemide
  • Blockage of reabsorption of sodium, potassium, and chloride ions by Furosemide
  • Antagonizing the action of aldosterone by Spironolactone

Dosage & Administration

  • Dosage range of Furosemide 20 and Spironolactone 50 mg
  • Flexible dosing based on patient's response
  • Higher dosage for previously stabilized patients
  • Not suitable for use in children
  • Slower excretion in the elderly

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Elevated levels of cardiac glycosides in the presence of Spironolactone
  • Possible hypokalemia with corticosteroids and Spironolactone combination
  • Potential reduction in diuretic effects when combined with indomethacin or NSAIDs
  • Enhanced ototoxicity of aminoglycoside antibiotics with Furosemide

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Side Effects

  • Headache and drowsiness
  • Gastrointestinal distress
  • Ataxia and mental confusion
  • Gynaecomastia and breast enlargement
  • Endocrine disorders including hirsutism and impotence
  • Transient increase in blood-urea-nitrogen concentrations
  • Hyponatremia and hyperkalemia
  • Dehydration and reduction in blood volume
  • Serious depletion of potassium and magnesium

Pregnancy & Lactation

  • Crossing the placental barrier by Spironolactone and its metabolites
  • Fetal abnormalities caused by Furosemide
  • Consideration of benefits versus potential hazards in pregnant women
  • Detection of metabolites in breast milk
  • Discontinuation of breast-feeding if treatment is essential

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, and hypovolemia
