Description
- Xevlin is indicated for infections in lower respiratory tract infections, upper respiratory tract infections, otitis media, skin and soft tissue infections, and sexually transmitted diseases
- Azithromycin is acid-stable and can be taken orally without protection from gastric acids
- Time to peak concentration in adults is 2.1 to 3.2 hours for oral dosage forms
- Plasma concentrations of azithromycin declined in a polyphasic pattern with a mean apparent plasma clearance of 630 mL/min and a terminal elimination half life of 68 hours
- Azithromycin acts by binding to the 50S ribosomal subunit of susceptible microorganisms and interfering with microbial protein synthesis
- Xevlin is well tolerated with a low incidence of side-effects

- Lower respiratory tract infections
- Upper respiratory tract infections
- Otitis media
- Skin and soft tissue infections
- Sexually transmitted diseases in men and women
Mode of Action
- Azithromycin is acid-stable
- Readily absorbed
- Greater absorption on an empty stomach
- High concentration in phagocytes
- Active transportation to the site of infection
- Large concentrations released during active phagocytosis
- High concentration of azithromycin in tissues
- Ion trapping and high lipid solubility
Dosage
- Oral administration in adults: 500 mg once daily for 3 days
- 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days
- For sexually transmitted diseases caused by Chlamydia trachomatis in adults, the dose is 1 gm given as a single dose or 500 mg once on day 1, followed by 250 mg once daily for next 2 days may also be given
- Oral administration in children: Dose based on body weight and specific conditions
- Azithromycin Injection (For IV Infusion only) in adults for the treatment of community-acquired pneumonia and pelvic inflammatory disease
- Not established in children or adolescents under 16 years
Administration
- Reconstitution procedure of suspension: Shake the bottle well, add boiled and cooled water up to the water mark of the bottle label, and shake until powder is completely mixed with water
- Azithromycin should be taken at least 1 hour before or 2 hours after a meal

Contraindications
- Hypersensitivity to Azithromycin or any other macrolide antibiotic
- Co-administration of ergot derivatives
- Patients with hepatic diseases
Side Effects
- Gastrointestinal side-effects including nausea, abdominal discomfort, vomiting, flatulence, diarrhea, and loose stools
- Allergic reactions such as rash or photosensitivity
- Rare reports of serious hypersensitivity reactions
- Reversible elevations in liver transaminases
- Rare cases of cholestatic jaundice
- Transient mild reductions in neutrophil counts
- Reversible hearing impairment in some patients with higher doses for prolonged periods
Pregnancy & Lactation
- Pregnancy Category B
- No evidence of harm to the fetus in animal reproduction studies
- No adequate and well controlled studies in pregnant women
- Caution in use during pregnancy if adequate alternatives are not available
- Unknown secretion in breast milk, caution in use during lactation
Precautions & Warnings
- Rare serious allergic reactions including angioneurotic oedema and anaphylaxis reported
- Some reactions resulted in recurrent symptoms and required a long period of observation and treatment
Use in Special Populations
- No dose adjustment needed in patients with mild renal impairment
- Caution in using azithromycin in patients with severe renal impairment
- Not to be used in patients with hepatic disease
- No evidence of effect on ability to drive or operate machinery
Overdose Effects
- No data on overdosage with Xevlin
- Typical symptoms of overdosage with macrolide antibiotics include hearing loss, severe nausea, vomiting, and diarrhea
