Indications
- Rheumatoid Arthritis

Pharmacology
- Autoimmune disease characterized by dysregulation of pro-inflammatory cytokines
- Tofacitinib is a partial and reversible janus kinase (JAK) inhibitor
- Inhibition of JAK-STAT pathway to decrease inflammatory response
Dosage & Administration
- Recommended dosage for Rheumatoid Arthritis: Tofacitinib 5 mg twice daily or 11 mg once daily
- Recommended dosage for Psoriatic Arthritis: Tofacitinib 5 mg twice daily or 11 mg once daily
- Recommended dosage for Ulcerative Colitis: Tofacitinib 10 mg twice daily for at least 8 weeks, then 5 or 10 mg twice daily
Interaction
- Potent inhibitors of Cytochrome P450 3A4 (CYP3A4) can affect dosage
- Concomitant medications that result in moderate inhibition of CYP3A4 and potent inhibition of CYP2C19 can affect dosage
- Potent CYP inducers may result in loss of or reduced clinical response
Side Effects
- Common adverse reactions: upper respiratory tract infections, headache, diarrhea, nasopharyngitis
Pregnancy & Lactation
- Pregnancy Category C: Use during pregnancy only if potential benefit justifies potential risk to fetus
- Not known whether Tofacitinib is excreted in human milk
Precautions & Warnings
- Avoid use during active serious infection
- Use with caution in patients at increased risk of gastrointestinal perforations
- Recommended laboratory monitoring for potential changes in various levels
- Avoid use with live vaccines
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Higher frequency of serious infection among Tofacitinib-treated subjects 65 years and older
- Half the total daily dosage recommended for patients with moderate and severe renal impairment or moderate hepatic impairment
