Indications
- Treatment of chronic hepatitis C (CHC) virus infection
- Combination treatment with other antiviral drugs
- Compensated liver disease
- Adult CHC patients coinfected with HIV
- Not to be used alone
Pharmacology
- Inhibitory action against respiratory syncytial virus
- Inhibitory action against influenza virus
- Inhibitory action against herpes simplex virus
- Mechanism of action not clear
- Interference with viral nucleic acid synthesis
Dosage & Administration
- Recommended duration of treatment: 24 to 48 weeks
- Assessment of virologic response after 24 weeks
- Treatment discontinuation consideration if HCV RNA not below limit by 24 weeks
- No safety and efficacy data for treatment longer than 48 weeks in untreated patients
- Recommended duration of treatment for relapse patients: 24 weeks
- No safety and efficacy data for treatment longer than 24 weeks in relapse patients
Interaction
- Nucleoside reverse transcriptase inhibitors
- Azathioprine
- Possible need for dose reduction or discontinuation
- Potential for severe pancytopenia with coadministration
Contraindications
- Pregnancy
- Risk of fetal harm
- Hemoglobinopathies (e.g. thalassemia major, sickle-cell anemia)
- Concomitant use with didanosine
Side Effects
- Psychiatric and central nervous system effects
- Severe ocular disorders
- Dental and periodontal disorders
- Growth inhibition in children and adolescents
- Similar adverse reactions in pediatric and adult subjects
Pregnancy & Lactation
- Pregnancy Category X
- Embryocidal and/or teratogenic effects
- Malformations in multiple organ systems
- Unknown excretion in human milk
- Potential for serious adverse reactions in nursing infants
Precautions & Warnings
- Strict avoidance in pregnancy and for 6 months post-treatment
- Requirement for negative pregnancy test prior to therapy
- Use of at least 2 forms of contraception
- Monthly pregnancy tests during therapy
- Avoidance of pregnancy for male patients and their partners
Use in Special Populations
- Safety and effectiveness not established in pediatric patients below age 3
- Greater risk of toxic reactions in patients with impaired renal function
