Indications
- Palliative treatment of advanced carcinoma of the breast or endometrium
- Treatment of anorexia, cachexia, or unexplained weight loss in patients with AIDS & cancer
Pharmacology
- Synthetic, antineoplastic and progestational drug
- Mechanism of action on endometrial carcinoma is unknown
- Inhibition of pituitary gonadotrophin production
- Decrease in estrogen secretion
- Direct cytotoxic effect on tumor cells
- Estrogen receptor mechanism
- Peak plasma concentrations occur 2 to 3 hours after a single oral dose
- Plasma half-life of 33 to 38 hours
- Approximately 66% excreted in urine and 20% in faeces
Dosage & Administration
- Tablet dosage: 160 mg/day for breast cancer
- Tablet dosage: 40-320 mg/day for endometrial carcinoma in divided doses
- At least 2 months of continuous treatment is considered adequate
- Oral suspension initial dosage: 800 mg/day (20 ml/day)
Interaction
- Possible interactions with concomitant medications have not been investigated
- Estrogens and Progestogens may cause fluid retention especially in high dosages or prolonged use
Contraindications
- Hypersensitivity to Megestrol Acetate or any component of the formulation
- Known or suspected pregnancy
Side Effects
- Weight gain
- Thromboembolic phenomena including thrombophlebitis and pulmonary embolism
- Glucocorticoid effects
- Nausea, dyspnea, tumor flare, hyperglycemia, glucose intolerance, alopecia, hypertension, carpal tunnel syndrome, mood changes, hot flashes, malaise, asthenia, lethargy, sweating and rash
Pregnancy & Lactation
- Pregnancy Category D
- Use during first four months of pregnancy not recommended
- Small amounts excreted in mother's milk
- Nursing should be discontinued during treatment
Precautions & Warnings
- Close surveillance for recurrent or metastatic cancer patients
- Use with caution in patients with a history of thromboembolic disease
- Exacerbation of preexisting diabetes with increased insulin requirements
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Insufficient data from clinical studies for patients 65 years and older
Overdose Effects
- No serious unexpected side effects reported up to 1600 mg/day
