Indications
- Symptomatic treatment of chronic constipation in adults when laxatives fail
Pharmacology
- Selective stimulator of 5-HT4 receptors
- No interaction with hERG channel or 5-HT1 receptors
- Stimulates motility by interacting specifically with 5-HT4 receptors
- Causes release of acetylcholine leading to propulsion of luminal content
Dosage & Administration
- Adults: 2 mg once daily with or without food
- Older people: Start with 1 mg once daily; dose can be increased to 2 mg once daily if needed
- Not for use in children and adolescents younger than 18 years
Interaction
- Low interaction potential with therapeutic concentrations
- Not expected to affect CYP-mediated metabolism of co-medicated medicinal products
- May have interactions with potent inhibitors of P-gp such as ketoconazole, verapamil, cyclosporine A, and quinidine
Contraindications
- Hypersensitivity to the active substance or excipients
- Renal impairment requiring dialysis
Side Effects
- Frequently reported adverse reactions include headache, abdominal pain, nausea, and diarrhea
- Adverse reactions occur predominantly at the start of therapy and usually disappear within a few days
Pregnancy & Lactation
- Not recommended during pregnancy
- Women of childbearing potential should use effective contraception during treatment
- Not recommended during breastfeeding based on animal studies
Precautions & Warnings
- Dose of 1 mg recommended in subjects with severe renal impairment
- Caution in prescribing to patients with severe hepatic impairment (Child-Pugh class C)
- Efficacy of oral contraceptives may be reduced in case of severe diarrhea
- Not recommended for patients with rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption
Use in Special Populations
- Dose adjustment for patients with severe renal impairment or severe hepatic impairment
- No dose adjustment required for patients with mild to moderate renal or hepatic impairment
Overdose Effects
- Symptoms may include headache, nausea, and diarrhea
- No specific treatment available
- Patient should be treated symptomatically and supportive measures instituted if required
