Indications
- Management of hypertension
- Long-term management of angina pectoris
- Management of hemodynamically stable patients with definite or suspected acute myocardial infarction
Pharmacology
- Beta1 selective (cardioselective) beta-adrenergic receptor antagonist
- Hydrophilic nature
- Competitively blocks beta1 (cardiac) receptors
- No effect on beta2-receptors
Dosage & Administration
- Initial dose for hypertension: 50 mg once daily
- Initial dose for angina pectoris: 50 mg once daily
- Initial dose for acute myocardial infarction: 5 mg intravenous injection over 5 minutes
Interaction
- Additive effect with catecholamine-depleting drugs
- Additive effect with calcium channel blockers
- Potential adverse effects with disopyramide and amiodarone
- Exacerbates rebound hypertension with the withdrawal of clonidine
- Decreased hypotensive effects with prostaglandin synthase inhibiting drugs
Contraindications
- Sinus bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Overt cardiac failure
- Hypersensitivity to atenolol or any of the drug product’s components

- Reversible mental depression
- Ischemic colitis
- Skin rashes and/or dry eyes
- Erythematous rash
Pregnancy & Lactation
- Pregnancy Category D
- Caution in nursing women due to clinically significant bradycardia in breast-fed infants
Precautions & Warnings
- Aggravates peripheral arterial circulatory disorders
- Use with caution in patients with impaired renal function
- Dose adjustment for elderly patients or those with renal impairment
- Evaluation of renal function for hypertension and myocardial infarction patients

- Excreted by the kidneys
- Adjusted dosage for elderly and renal-impaired patients
- Maximum recommended dosages based on creatinine clearance
Overdose Effects
- Lethargy
- Disorder of respiratory drive
- Wheezing
- Sinus pause
- Bradycardia
- Congestive heart failure
- Hypotension
- Bronchospasm
- Hypoglycemia
- Management may require intensive support care
