Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with ascites
  • Swelling due to excess fluid retention
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Pharmacology

  • Complementary mechanisms and sites of action for Spironolactone and Furosemide
  • Additive or synergistic diuretic effects
  • Inhibition of sodium, potassium, and chloride ion reabsorption

Dosage & Administration

  • Dosage range of Furosemide 20 and Spironolactone 50 mg tablets

Interaction

  • Increased risk of hyperkalemia when taken with ACE inhibitors or potassium salts
  • Elevated levels of cardiac glycosides when combined with Spironolactone
  • Possible hypokalemia with corticosteroids
  • Reduced or abolished diuretic effects when used with indomethacin or NSAIDs
  • Potential increase in ototoxicity with aminoglycoside antibiotics
  • Diminished natriuretic and anti-hypertensive effect when used with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Side Effects

  • Headache, drowsiness, gastrointestinal distress
  • Ataxia, mental confusion, skin rashes
  • Gynaecomastia, breast enlargement
  • Endocrine disorders, hirsutism, voice deepening, menstrual irregularities, impotence
  • Transient increase in blood-urea-nitrogen concentrations
  • Hyponatremia, hyperkalemia
  • Dehydration, reduction in blood volume, circulatory collapse
  • Potassium and magnesium depletion, cardiac arrhythmias

Pregnancy & Lactation

  • Crossing of placental barrier by Spironolactone and its metabolites
  • Possibility of Furosemide causing fetal abnormalities
  • Need for careful consideration of risks and benefits during pregnancy and lactation
  • Excretion of Furosemide in breast milk

Precautions & Warnings

  • Caution in patients with electrolyte deficiency, diabetes, enlarged prostate, hypotension, hypovolemia
