Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibition of reuptake of neurotransmitters (serotonin and noradrenaline)
  • Modifies transmission of pain impulses by activating serotonergic and noradrenergic pathways
  • Stimulation of mu-opioid receptors

Dosage & Administration

  • Usual doses of 50 to 100 mg every four to six hours
  • Initial dose of 100 mg for acute pain
  • Total daily dosage by mouth should not exceed 400 mg
  • Sustained release capsule or tablet to be taken every 12 hours
  • Injection dosage of 50-100 mg every 4 to 6 hours
  • Suppository administered rectally with usual dose of 100 mg Tramadol Hydrochloride 6 hourly
  • Total daily dosage of Tramadol Suppository varies for different pain conditions

Interaction

  • Contraindicated with monoamine oxidase (MAO) inhibitors
  • Carbamazepine interaction

Contraindications

  • Hypersensitivity to the drug
  • Acute intoxication with specified substances

Side Effects

  • Common: dizziness/vertigo, nausea, constipation, headache, somnolence, vomiting
  • Less common: malaise, allergic reaction, weight loss, GI bleeding, hepatitis

Pregnancy & Lactation

  • Safety not established in pregnancy
  • Tramadol crosses the placenta
  • Not recommended during breastfeeding

Precautions & Warnings

  • Risk of respiratory depression
  • Not recommended for opioid-dependent patients
  • Caution with concomitant CNS depressants
  • Use with caution in patients taking MAO inhibitors
  • Caution in patients with increased intracranial pressure or head injury

Use in Special Populations

  • Dose for children from the age of 1 year
  • Suppository not recommended for children and adolescents below the age of 14 years
  • SR Capsules not studied in children
