Indications

  • Lowering blood glucose in noninsulin dependent (Type II) diabetes mellitus
  • Adjunct to diet and exercise
  • Combination with metformin
  • Combination with insulin

Pharmacology

  • Sulfonylurea antidiabetic agent
  • Stimulates insulin release from pancreatic beta cells
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Improves glucose uptake

Dosage

  • Initial dose of 1 mg once daily
  • Dose titration based on glucose monitoring
  • Usual dose range of 1 to 4 mg daily
  • Timing and distribution of doses decided by physician

Administration

  • Must be swallowed with sufficient liquid

Interaction

  • May potentiate hypoglycaemic action with ACE inhibitors, aminosalicylic acid, fluconazole, etc.
  • May attenuate hypoglycaemic action with acetazolamide, barbiturates, calcium channel blockers, etc.
  • Beta-blockers, clonidine, and reserpine may mask hypoglycaemic symptoms

Contraindications

  • Not suitable for insulin dependent diabetes mellitus
  • Not for diabetic ketoacidosis or diabetic coma
  • Not for patients hypersensitive to Glimepiride, other sulfonylureas, or sulfonamides

  • Diarrhoea
  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin necessary
  • Not to be taken by breastfeeding women

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Careful monitoring required
  • Prompt control of hypoglycaemia with immediate intake of carbohydrates

Overdose Effects

  • Hypoglycaemia
  • Mild symptoms treated with oral glucose
  • Severe cases require immediate hospitalization
  • Continuous monitoring for 24 to 48 hours
