Indications
- Treatment of essential (primary) hypertension
- Treatment of secondary hypertension
- Treatment of left ventricular failure
- Treatment of Raynaud's phenomenon and Raynaud's disease
- Adjunct in the symptomatic treatment of urinary obstruction caused by benign prostatic hyperplasia
Pharmacology
- Causes decrease in total peripheral vascular resistance
- Selective inhibition of postsynaptic alpha-1-adrenoreceptors in vascular smooth muscle
- Blood pressure lowering in supine and standing positions
- Reduction in left ventricular filling pressure
- Reduction in cardiac impedance
- Improves urinary pressure profile in men
Dosage & Administration
- Therapy initiated with a low starting dose
- Dosage adjusted according to the patient's individual toleration
- Maintenance dosage given as a twice or three times daily regimen
- Recommended starting dosage for left ventricular failure
- Use daily maintenance dosage for left ventricular failure
- Recommended starting dosage for Raynaud's phenomenon and Raynaud's disease
- Usual daily maintenance dosage for Raynaud's phenomenon and Raynaud's disease
- Recommended starting dose for benign prostatic hyperplasia
- Recommended starting dose for Prazosin XR Extended-Release Tablets
- Maintenance dose for Prazosin XR Extended-Release Tablets
- Patients controlled on Prazosin Tablets alone or in combination with other antihypertensive medications may be switched to Prazosin XR Extended Release Tablets
Interaction
- No adverse drug interaction observed in clinical experience
- Administered without adverse drug interaction with cardiac-glycosides, hypoglycemic agents, tranquilizers and sedatives, antiarrhythmic agents, analgesic, antipyretic, and anti-inflammatory agents
Contraindications
- Contraindicated in patients with known sensitivity to quinazolines
Side Effects
- Common reactions include dizziness, headache, drowsiness, lack of energy, weakness, palpitations, and nausea
- Other reactions associated with Minipress XL therapy
- Nervousness, vertigo, hallucinations, depression, rash, urinary frequency, impotence, incontinence, priapism, blurred vision, dry mouth, fever, arthralgia
Pregnancy & Lactation
- No teratogenic effects seen in animal testing
- Safety of Prazosin use during pregnancy not yet established
- Excreted in small amounts in human milk
Precautions & Warnings
- Postural hypotension reported with the commencement of therapy
- Recommendation for careful monitoring of blood pressure
- Left ventricular failure patients may experience a clinical efficacy diminishment after several months of treatment
Use in Special Populations
- Not recommended for the treatment of children under the age of 12 years
- Not recommended for left ventricular failure due to mechanical obstruction
- Adequate data not yet available for left ventricular failure due to a recent myocardial infarction
Overdose Effects
- Accidental ingestion of at least 50 mg of Minipress XL in a two-year child resulted in profound drowsiness and depressed reflexes
- No decrease in blood pressure was noted
