Indications
- Treatment of adult patients with moderate to severely active Rheumatoid Arthritis
- Inadequate response to one or more tumor necrosis factor (TNF) antagonist therapies
Pharmacology
- Selective and reversible inhibitor of Janus kinase JAK1 and JAK2
- Transduction of intracellular signals from cell surface receptors for cytokines and growth factors
- Involvement in haematopoiesis, inflammation, and immune function
- Reduction of phosphorylation and activation of signal transducers and activators of transcription (STATs)
Dosage & Administration
- Recommended adult dose of 2 mg once daily
- May be used as monotherapy or in combination with Methotrexate or other Disease-modifying antirheumatic drugs (DMARDS)
- Can be given orally with or without food
- Safety and effectiveness in pediatric patients not established
- Dose adjustment for elderly patients with decreased renal function
- No dose adjustment in patients with mild or moderate hepatic impairment
- Not recommended for use in patients with estimated GFR of less than 60 mL/min/I .73 m2
Interaction
- Increased exposure when co-administered with strong OAT3 inhibitors (e.g., probenecid)
- Not studied in combination with other JAK inhibitors or with biologic DMARDS
Contraindications
- Avoid initiation or interrupt in patients with hemoglobin less than 8 g/dL
- Avoid initiation or interrupt in patients with an Absolute Lymphocyte Count less than 500 cells/mm3
- Avoid initiation or interrupt in patients with an Absolute Neutrophil Count less than 1000 cells/mm3
Side Effects
- Most commonly reported adverse drug reactions: increased LDL cholesterol, upper respiratory tract infections, nausea
Pregnancy & Lactation
- Contraindicated during pregnancy

Precautions & Warnings
- Avoid use in patients with an active, serious infection, including localized infections
- Not to be given to patients with active TB
- Consider risks and benefits prior to initiating therapy in patients with a known malignancy
- Use with caution in patients at increased risk of thrombosis
- Use with caution in patients at increased risk for gastrointestinal perforation
- To be dispensed only by the prescription of a registered physician
Overdose Effects
- Patient should be monitored for signs and symptoms of adverse reactions
- Patients who develop adverse reactions should receive appropriate treatment
