Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis
  • Swelling due to excess fluid retention
  • Hyperaldosteronism
  • Resistant edema
  • Collection of fluid in the abdominal cavity

Pharmacology

Spironolactone and Furosemide have complementary mechanisms and sites of action, producing synergistic diuretic effects.

Dosage & Administration

  • Furosemide 20 and spironolactone 50 mg: 1 to 4 tablets daily
  • Furosemide 40 and spironolactone 50 mg: 1 to 2 tablets daily
  • Not suitable for use in children
  • Excreted more slowly in the elderly

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Increased levels of cardiac glycosides when taken with digoxin
  • Hypokalemia with corticosteroids
  • Reduced or abolished effects when used with indomethacin
  • Increased ototoxicity of aminoglycoside antibiotics with Furosemide
  • Reduced natriuretic and anti-hypertensive effect with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalaemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Pregnancy & Lactation

  • Pregnancy: Requires weighing benefit against possible hazards
  • Lactation: Metabolites detected in breast milk; consider alternative feeding method

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, and hypovolemia
