Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis
- Swelling due to excess fluid retention
- Hyperaldosteronism
- Resistant edema associated with secondary hyperaldosteronism
Pharmacology
- Complementary mechanisms of spironolactone and furosemide
- Additive or synergistic diuretic effect
- Inhibition of Na+/K+/2Cl- co-transporter
- Blockage of reabsorption of sodium, potassium, and chloride ions
- Potassium loss
- Inhibition of aldosterone action
- Increased sodium excretion
- Reduced potassium loss induced by furosemide
Dosage & Administration
- 1 to 4 tablets daily of furosemide 20 and spironolactone 50 mg
- 1 to 2 tablets daily of furosemide 40 and spironolactone 50 mg for stabilized patients requiring higher dosage
- Not suitable for use in children
- Slower excretion in the elderly
Interaction
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Elevated levels of cardiac glycosides such as digoxin in the blood
- Possible digitalis toxicity
- Hypokalemia with corticosteroids
- Reduced or abolished effects when used with indomethacin and possibly other NSAIDs
- Increased ototoxicity of aminoglycoside antibiotics
- Reduced natriuretic and anti-hypertensive effect with simultaneous use of sucralfate
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalemia
- Addison's disease
- Hypersensitivity to spironolactone, furosemide, or sulphonamides

- Hyponatremia
- Excessive diuresis leading to dehydration
- Reduction in blood volume
- Cardiac arrhythmias
- Thrombosis
- Embolism
Pregnancy & Lactation
- Crossing the placental barrier
- Possible fetal abnormalities
- Need for careful assessment of benefit versus hazards in pregnant women
- Detection of metabolites in breast milk
- Consideration of alternative feeding method if essential use of spironolactone is required
- Excretion in breast milk
- Discontinuation of breastfeeding if essential to use furosemide
Precautions & Warnings
- Caution in patients liable to electrolyte deficiency
- Use with caution in diabetes, enlarged prostate, hypotension, hypovolemia
