Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis with ascites
- Swelling due to fluid retention
- Hyperaldosteronism
- Resistant edema with hyperaldosteronism
Pharmacology
- Complementary mechanisms and sites of action for spironolactone and furosemide
- Additive or synergistic diuretic effect
- Furosemide inhibits Na+/K+/2Cl- co-transporter
- Spironolactone inhibits reabsorption of sodium
- Characteristically induces potassium loss
- Spironolactone reduces excess loss of potassium induced by furosemide
Dosage & Administration
- Furosemide 20mg and spironolactone 50mg tablet
- Furosemide 40mg and spironolactone 50mg tablet
- Dosage range of Furosemide 20-80mg and spironolactone 50-200mg
- Use not suitable for children
- Slower excretion in elderly
Interaction
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Increased levels of cardiac glycosides with spironolactone
- Potential digitalis toxicity
- Hypokalemia risk with corticosteroids
- Reduced diuretic effects with indomethacin or NSAIDs
- Increased ototoxicity with aminoglycoside antibiotics
- Reduced natriuretic effect with sucralfate
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalemia
- Addison's disease
- Hypersensitivity to spironolactone or furosemide

- Headache and drowsiness
- Gastrointestinal distress
- Ataxia and mental confusion
- Skin rashes
- Gynaecomastia and breast enlargement
- Endocrine disorders
- Increase in blood-urea-nitrogen concentrations
- Mild acidosis
- Hyponatremia and hyperkalemia
- Dehydration and reduction in blood volume
- Cardiac arrhythmias
- Vascular thrombosis and embolism
Pregnancy & Lactation
- Spironolactone crosses placental barrier
- Possible hazards to mother and fetus
- Fetal abnormalities with furosemide
- Use in women of childbearing age requires appropriate contraceptive measures
- Discontinue breastfeeding if treatment is essential
Precautions & Warnings
- Caution in patients with electrolyte deficiency
- Use with caution in diabetes, enlarged prostate, hypotension, and hypovolemia
