Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis with ascites
- Swelling due to excess fluid retention
- Hyperaldosteronism
- Resistant edema associated with secondary hyperaldosteronism
Pharmacology
- Complementary mechanisms of action
- Additive diuretic effect
- Inhibition of Na+/K+/2Cl- co-transporter
- Blockage of sodium and potassium reabsorption
- Potassium loss induced by Furosemide
- Inhibition of aldosterone action by Spironolactone
- Increased sodium excretion
Dosage & Administration
- 20 to 80 mg of Furosemide daily
- 50 to 200 mg of Spironolactone daily
- Usage in children not suitable
- Slower excretion in the elderly
Interaction
- Increased risk of hyperkalemia with ACE inhibitors
- Potassium salts interaction
- Elevated levels of cardiac glycosides
- Digitalis toxicity risk
- Hypokalemia with corticosteroids
- Reduced diuretic effects with NSAIDs
- Increase in ototoxicity with aminoglycoside antibiotics
- Reduced natriuretic effect with sucralfate
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalemia
- Addison's disease
- Hypersensitivity to Spironolactone or Furosemide
Side Effects
- Headache and drowsiness
- Gastrointestinal distress
- Ataxia and mental confusion
- Gynaecomastia
- Endocrine disorders
- Transient increase in blood-urea-nitrogen
- Mild acidosis
- Hyponatremia and hyperkalemia
- Dehydration and reduction in blood volume
- Serious depletion of potassium and magnesium

- Crossing placental barrier
- Potential fetal abnormalities
- Excretion in breast milk

- Caution in patients liable to electrolyte deficiency
- Use with caution in diabetes and hypotension
