Indications
- Reduction of myocardial infarction (MI)
- Reduction of stroke in patients with non-ST-segment elevation ACS
- Reduction of MI and stroke in patients with acute ST-elevation myocardial infarction (STEMI)
- Reduction of MI and stroke in patients with recent MI, recent stroke, or established peripheral arterial disease
Pharmacology
Irreversible binding of active metabolite to the P2Y12 class of ADP receptors on platelets
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg once daily orally for recent MI, recent stroke, or established peripheral arterial disease
- Given orally with or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors (omeprazole or esomeprazole)
- Avoid concomitant use with Repaglinide as it increases plasma concentrations
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: bleeding, diarrhea, gastrointestinal discomfort, hemorrhage, skin reactions
- Rare: acquired hemophilia, anemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
Use during pregnancy only if clearly needed, unknown excretion in human breast milk
Precautions & Warnings
- Impaired metabolism by genetic variations in CYP2C19
- Increased risk of bleeding, reduced antiplatelet activity with CYP2C19 inhibitors
- Increased risk of cardiovascular events upon discontinuation
- Reported cases of thrombotic thrombocytopenic purpura (TTP)
- Reported hypersensitivity reactions
Use in Special Populations
Safety and effectiveness in pediatric populations have not been established
Overdose Effects
May lead to bleeding complications, potential restoration of clotting ability with platelet transfusion
