Indications
- Symptomatic relief of Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Pain and signs of inflammation associated with acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1 at doses up to 150 mg daily
Dosage & Administration
- Recommended dose for Osteoarthritis is 30 mg once daily, may increase to 60 mg if insufficient relief
- Recommended dose for Rheumatoid arthritis and Ankylosing spondylitis is 90 mg once daily
- Recommended dose for Acute gouty arthritis is 120 mg once daily for 8 days
- Recommended dose for Postoperative dental surgery pain is 90 mg once daily, limited to a maximum of 3 days
Interaction
- Interaction with oral anticoagulants, diuretics, ACE inhibitors, angiotensin II antagonists, acetylsalicylic acid, ciclosporin, tacrolimus, and lithium
- Take without regards to meals
Contraindications
- Hypersensitivity to the active substance or any of the excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Pregnancy and lactation
- Severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease, congestive heart failure, hypertension, established ischaemic heart disease, peripheral arterial disease, or cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Less commonly dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Very rarely confusion and hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk, not recommended for breastfeeding women
Precautions & Warnings
- Caution in patients at risk of developing gastrointestinal complications with NSAIDs, significant risk factors for cardiovascular events
- Monitoring of renal function in patients with impaired renal function
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function, discontinuation in case of hepatic insufficiency or signs of hypersensitivity
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day for 21 days did not result in significant toxicity
- Employ usual supportive measures in case of overdose
