Indications

  • Acute and delayed nausea and vomiting
  • Uncontrolled nausea and vomiting
  • Chemotherapy-induced nausea and vomiting (CINV)
  • Radiotherapy-induced nausea and vomiting (RINV)
  • Post-operative & Post-discharge nausea and vomiting (PONV & PDNV)

Pharmacology

  • 5-HT3 receptor antagonist
  • Strong binding affinity for 5-HT3 receptor
  • Little or no affinity for other receptors
  • Inhibition of serotonin actions
  • Influence on emetic response
  • Selective participation in the emetic response
  • Linear dose-proportional pharmacokinetics
  • Metabolism via kidney and liver CYP enzymes
  • Excretion via urine
  • Longer half-life
  • Greater 5-HT3 receptor binding affinity

Dosage & Administration

  • Adult tablet dosage: 0.5 mg daily
  • Adult IV dosage: 0.075 mg administered over 10 seconds
  • Specific dosages for different nausea and vomiting causes
  • Children dosage: single IV dose at 20 mcg/kg body weight
  • Maximum children dose: 1.5 mg

Interaction

  • Safety with various medication classes
  • No inhibition of antitumor activity
  • No significant pharmacokinetic interactions with metoclopramide
  • Low potential for drug interactions

Contraindications

  • Hypersensitivity to the drug or any of its components

Side Effects

  • Common adverse reactions: headaches and constipation

Pregnancy & Lactation

  • Pregnancy category 'B'
  • Unknown excretion in breast milk

Use in Special Populations

  • No dosage adjustment in elderly patients >65 years of age
  • Specific dosage for children (1 month to 10 years)
  • No dosage adjustment in patients with renal and hepatic dysfunction

Overdose Effects

  • No known antidote to Palotic
  • Management with supportive care
