Indications
- Adjunct to diet and exercise in type 2 diabetes mellitus patients
- Monotherapy with glimepiride or metformin does not result in adequate glycemic control
- Replacement of combination therapy of glimepiride and metformin
Pharmacology
- Decreases blood glucose concentration
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Improves glucose uptake
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
- Substrate for transporters OCT1 and OCT2
Dosage
- Governed by the desired blood glucose level
- Must be lowest sufficient to achieve desired metabolic control
- Glucose levels in blood and urine must be measured regularly
- Do not correct mistakes by taking a larger dose
- Glimepiride requirements may fall as treatment proceeds
- The highest recommended dose per day
- Starting dose should not exceed daily doses of glimepiride or metformin already being taken
- When switching from combination therapy, administer on basis of current dosage
Administration
- Must be swallowed whole
- Not crushed or chewed
Interaction
- Metabolized by cytochrome P450 2C9 (CYP2C9)
- Potentiation and hypoglycemia with certain drugs
- Weakening and raised blood glucose levels with certain drugs
- May interact with alcohol
- Bile acid sequestrant reduces absorption
Contraindications
- Hypersensitivity to glimepiride, metformin, or any excipients
- Pregnant women
- Breastfeeding women
- Severe impairment of liver function
- Dialysis patients
- Hypersensitivity to metformin or excipients
- Acute metabolic acidosis
- Severe renal failure or dysfunction
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
- Hepatic insufficiency
- Acute alcohol intoxication
- Lactation

Pregnancy & Lactation
- Glimepiride not to be taken during pregnancy
- Metformin not recommended for pregnancy
- Glimepiride not for breastfeeding women
- Metformin excreted into milk, decision should be made
Precautions & Warnings
- Risk of hypoglycemia in initial weeks of treatment
- Increased risk in presence of hypoglycemia risk factors
- Severe hypoglycemia requires immediate treatment
- G6PD-deficiency caution
- Regular monitoring of TSH levels in hypothyroidism
- Long-term treatment associated with decrease in vitamin B12 levels
Use in Special Populations
- Insufficient data for pediatric use
- Assessment of GFR before initiation of treatment
- Maximum daily dose in relation to declining renal function
Overdose Effects
- Severe life-threatening hypoglycemia
- Lactic acidosis
- Pancreatitis
