Pharmacological Class
- Sodium-glucose Cotransporter-2 (SGLT2) Inhibitors
Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
- To reduce the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease
Mode of Action
- Inhibition of SGLT2 co-transporters resulting in reduced renal absorption of glucose, leading to increased glucose excretion, reduced hyperglycemia, weight loss, and lowered blood pressure
Dosage Administration
- Recommended dose of 10 mg once daily in the morning, with or without food
- Dose may be increased to 25 mg once daily in patients tolerating it
- Correction of volume depletion prior to initiation recommended
Interactions
- Increased urine volume when co-administered with diuretics
- Increased risk of hypoglycemia when co-administered with insulin or insulin secretagogues
- Not recommended to monitor glycemic control with urine glucose tests or 1,5-AG assay in patients taking SGLT2 inhibitors
Contraindications
- History of serious hypersensitivity reaction to Empagliflozin or any of its ingredients
- Severe renal impairment
- End-stage renal disease
- Dialysis

- Female genital mycotic infections
- Dehydration
- Hypotension
- Weakness
- Dizziness
- Increased thirstiness
Pregnancy Lactation
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Not recommended when breastfeeding
Precautions Warnings
- Assessment of renal function recommended prior to initiation and periodically thereafter
- Not initiated in patients with an eGFR less than 45 ml/min/1.73 m²
- No dose adjustment needed in patients with an eGFR greater than or equal to 45 ml/min/1.73 m²
Overdose Effects
- Usual supportive measures should be employed
- Hemodialysis has not been studied for removal of Emjard
