Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Reduction in risk of cardiovascular death in type 2 diabetes with established cardiovascular disease
Pharmacology
- Sodium glucose co-transporter-2 (SGLT-2) inhibitor
- Glucuretic effect
- Reduces renal threshold for glucose
- Assists weight loss
- Reduces blood pressure
Dosage & Administration
- Recommended dose: 10 mg once daily
- Dose may be increased to 25 mg once daily in tolerating patients
- Recommended to take in the morning with or without food
- Correction of volume depletion is recommended before initiation
Interaction
- Increased urine volume with diuretics co-administration
- Increased risk of hypoglycemia with insulin or insulin secretagogues co-administration
- Not recommended to monitor glycemic control with urine glucose tests
- Unreliable 1,5-AG assay for glycemic control in SGLT2 inhibitors patients
Contraindications
- History of serious hypersensitivity reaction to Empagliflozin or its ingredients
- Severe renal impairment
- End-stage renal disease
- Dialysis

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Not recommended in breastfeeding
Precautions & Warnings
- Renal function assessment recommended before initiation and periodically thereafter
- Initiation not recommended in patients with eGFR less than 45 ml/min/1.73 m2
- No dose adjustment needed in patients with eGFR greater than or equal to 45 ml/min/1.73 m2
