Indications
- Adjunct treatment for type 2 diabetes mellitus in adults
- Combined use with Metformin or other diabetes medications
- Treatment for patients already using Empagliflozin and Metformin separately
Pharmacology
- Inhibitor of Sodium-Glucose Co-Transporter 2 (SGLT2)
- Reduces renal reabsorption of glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Increases urinary glucose excretion
Dosage & Administration
- Dosage individualized based on effectiveness and tolerability
- Take twice daily with meals
- Gradual dose escalation for reduced gastrointestinal side effects
Interaction
- Increased urine volume with diuretics
- Risk for hypoglycemia with insulin or insulin secretagogues
- Monitoring glycemic control with urine glucose tests not recommended
Contraindications
- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis
- Severe renal failure
- Conditions altering renal function
- Tissue hypoxia
- Hepatic impairment
- Acute alcohol intoxication

Precautions & Warnings
- Lactic Acidosis
- Hypotension
- Ketoacidosis
- Acute kidney injury
- Increased risk for urinary tract infections
- Hypoglycemia risk
- Vitamin B12 Deficiency
- Increased LDL-C
- No established evidence for macrovascular risk reduction
Overdose Effects
- No toxicity observed with single doses of Empagliflozin up to 800 mg
- No hypoglycemia observed with Metformin doses up to 85 g
- Lactic acidosis is a medical emergency and must be treated in hospital
- Haemodialysis is the most effective method to remove lactate and Metformin
