Indications
- Treatment of chronic hepatitis B virus infection in adults with compensated liver disease
Pharmacology
- Tenofovir alafenamide is a nucleoside analog reverse transcriptase inhibitor
- Enters primary hepatocytes by passive diffusion
- Converted to tenofovir through hydrolysis in primary hepatocytes
- Phosphorylated to tenofovir diphosphate, which inhibits HBV replication
Dosage & Administration
- Recommended dosage of 25 mg (one tablet) taken orally once daily with food in adults
- No dosage adjustment in patients with estimated creatinine clearance greater than or equal to 15 ml per minute
- Not recommended in patients with decompensated hepatic impairment
Interaction
- Is a substrate of P-glycoprotein and BCRP
- Drugs that strongly affect P-gp and BCRP activity may lead to changes in absorption
- Coadministration with drugs that inhibit P-gp and BCRP may increase plasma concentration
Side Effects
- Most common side effects include headache, stomach pain, tiredness, cough, nausea, back pain
Pregnancy & Lactation
- Prevalence of birth defects in live births following exposure to TAF-containing regimens
- Tenofovir alafenamide presence and effects on breastfed infant unknown
Precautions & Warnings
- Discontinuation may result in severe acute exacerbations of hepatitis B
- Not recommended for treatment of HIV-1 infection
- Postmarketing cases of renal impairment reported with TAF-containing products
- Lactic acidosis and severe hepatomegaly with steatosis reported with nucleoside analogs
Overdose Effects
- Monitor patient for evidence of toxicity
- Efficiently removed by hemodialysis
