Pharmacology
- Additive antihypertensive effects of Bisoprolol and Hydrochlorothiazide
- Bisoprolol Fumarate is a β1-selective adrenoceptor blocking agent
- Hydrochlorothiazide is a benzothiadiazine diuretic
- Mechanisms of electrolyte reabsorption affected by thiazides
Dosage & Administration
- Effective doses range for Bisoprolol (2.5 to 40 mg) and Hydrochlorothiazide (12.5 to 50 mg)
- Initial therapy with one 2.5/6.25 mg tablet once daily
- Subsequent titration at 14-day intervals
- Maximum recommended dose of 20/12.5 mg once daily
Interaction
- Potentiation of action of other antihypertensive agents
- Avoid combination with other beta-blocking agents
- Cautious use with myocardial depressants or inhibitors of AV conduction
- Increased risk of bradycardia with concomitant use of digitalis glycosides and beta-blockers
Contraindications
- Cardiogenic shock
- Overt cardiac failure
- Second or third degree AV block
- Marked sinus bradycardia
- Anuria
- Hypersensitivity to either component or other sulfonamide-derived drugs

Precautions & Warnings
- Hyperuricemia or acute gout may be precipitated in certain patients
- Warning signs of fluid and electrolyte imbalance
- Hypokalemia may develop
- Gradual withdrawal over a period of about 2 weeks
Overdose Effects
- Signs of overdose: bradycardia, hypotension, lethargy
- Severe overdoses may lead to delirium, coma, convulsions, and respiratory arrest
- Acute loss of fluid and electrolytes
- Cardiovascular, neuromuscular, gastrointestinal, renal symptoms
