Indications
- Adjunct in the treatment of all forms of parkinsonism
- Useful as adjuvant therapy with levodopa

Description
- Exerts inhibitory effect on parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for central muscarinic receptors in the cerebral cortex
- Lower affinity for peripheral muscarinic receptors
Dosage & Administration
- Individualized dosage
- Initial dose should be low and increased gradually
- Postencephalitic patients may prefer taking it after meals
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensification of anticholinergic effects with MAOIs and tricyclic antidepressants

- Narrow angle glaucoma

Precautions & Warnings
- Gonioscope evaluation prior to therapy
- Monitoring of intraocular pressures
- Caution in hot weather and with other atropine-like drugs
- Possibility of anhidrosis and hyperthermia
- Risk of Neuroleptic Malignant Syndrome
Overdose Effects
- Central symptoms of atropine intoxication
- Peripheral signs of parasympathetic blockade
- Neuropsychiatric signs
- Stupor, coma, paralysis, cardiac and respiratory arrest, and death in severe cases
- Treatment involves symptomatic and supportive therapy, gastric lavage, diazepam administration, respiratory support, fluid replacement, and acid-balance maintenance
