Indications

  • Treatment of Hypertension

Pharmacology

  • Bisoprolol Fumarate and Hydrochlorothiazide used individually and in combination for hypertension treatment
  • Antihypertensive effects of agents are additive
  • Incidence of hypokalemia with Bisoprolol Fumarate and Hydrochlorothiazide 6.25 mg combination is significantly lower than with Hydrochlorothiazide 25 mg
  • Bisoprolol Fumarate is a β1-selective (cardioselective) adrenoceptor blocking agent
  • Hydrochlorothiazide is a benzothiadiazine diuretic affecting renal tubular mechanisms of electrolyte reabsorption and increasing excretion of sodium and chloride

Dosage & Administration

  • Effective doses of Bisoprolol 2.5 to 40 mg and Hydrochlorothiazide 12.5 to 50 mg
  • Initial therapy with one 2.5/6.25 mg tablet once daily
  • Subsequent titration with maximum recommended dose 20/12.5 mg once daily
  • Therapy substitution and guided by clinical effect

Interaction

  • Potentiation of action of other antihypertensive agents
  • Not to be combined with other beta-blocking agents
  • Cautious use with myocardial depressants or AV conduction inhibitors
  • Increased risk of bradycardia with concomitant use of digitalis glycosides and beta-blockers

Contraindications

  • Cardiogenic shock
  • Overt cardiac failure
  • Second or third degree AV block
  • Marked sinus bradycardia
  • Anuria
  • Hypersensitivity to either component or other sulfonamide-derived drugs

Side Effects

  • Mild and transient side effects
  • Side effects may include fatigue, dizziness, headache, bradycardia, arrhythmia, peripheral ischemia, chest pain, palpitations, cold extremities, orthostatic hypotension, gastrointestinal disturbances, rhinitis, pharyngitis

Pregnancy & Lactation

  • Pregnancy Category C
  • Use during pregnancy only if potential benefit justifies risk to fetus
  • Thiazides excreted in human breast milk

Precautions & Warnings

  • Possibility of hyperuricemia or acute gout with thiazide diuretics
  • Warning signs or symptoms of fluid and electrolyte imbalance
  • Hypokalemia risk and gradual withdrawal of therapy

Overdose Effects

  • Most frequently observed signs expected with overdose of a beta-blocker are bradycardia and hypotension
  • Lethargy, delirium, coma, convulsions, and respiratory arrest reported with severe overdoses
  • Acute loss of fluid and electrolytes in thiazide diuretic overdose
