Indications
- Symptomatic relief of Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Pain and signs of inflammation associated with acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Selective inhibition of COX-2 decreases potential for GI toxicity
- Did not inhibit gastric prostaglandin synthesis
Dosage & Administration
- Recommended doses for different conditions: 30mg, 60mg, 90mg, 120mg once daily
- Shortest duration and lowest effective daily dose is recommended
Interaction
- With medicine: Oral anticoagulants, Diuretics, ACE inhibitors, Acetylsalicylic Acid, Ciclosporin and tacrolimus, Lithium
- With food & others: Take without regards to meals
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or active GI bleeding
- Patients with specific allergic-type reactions
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min, children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, transient ischaemic attack, chest pain
- Flushing, cough, dyspnoea, epistaxis, anxiety
- Myalgia, arthralgia, confusion, hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution in patients at risk of GI complications with NSAIDs
- Caution in patients with significant risk factors for cardiovascular events
- Monitoring of renal function should be considered
- Caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension or pre-existing edema
- Monitoring of liver function should be considered
- Caution in co-administration with warfarin or other oral anticoagulants
Overdose Effects
- Single doses up to 500mg and multiple doses up to 150mg/day did not result in significant toxicity
- Usual supportive measures recommended in case of overdose
