Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition

Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive properties
- Inhibition of late phase allergic reactions
- Reduction of mast cell density
- Reduction of cytokine production
Dosage
- Application of a thin layer twice daily for adults, elderly and children over 1 year
- Enhancement of effect by occluding treatment area with polythene film
- Super-high potency topical corticosteroid
- Maximum weekly dose of 50 gm/week
- Limited treatment to 2 consecutive weeks
- Application to dry scalp once a day
- Treatment should be limited to 4 consecutive weeks for shampoo
- Total dosage of shampoo should not exceed 50 g per week
- Total dose applied should not exceed 50 ml weekly for scalp solution
- Therapy should be discontinued if no response is noted after one week
Administration
- Route of administration: Cutaneous
- Creams for moist or weeping surfaces
- Ointments for dry, lichenified or scaly lesions
Interaction
- Inhibition of CYP3A4 by co-administered drugs like ritonavir, itraconazole
Contraindications
- Rosacea, acne vulgaris and perioral dermatitis
- Hypersensitivity to the preparation
- Primary cutaneous viral infections
- Dermatoses in children under one year of age
- Not indicated in the treatment of primarily infected skin lesions caused by fungi or bacteria
Side Effects
- Features of hypercorticism with prolonged use of large amounts
- Local atrophic changes in the skin with prolonged and intensive treatment
- Reports of pigmentation changes and hypertrichosis
- Hypersensitivity signs leading to application cessation
Pregnancy & Lactation
- Limited data from the use in pregnant women
- Caution in administration to nursing women
Precautions & Warnings
- Avoid long-term continuous topical therapy, particularly in infants and children
- Risk of atrophic changes after prolonged treatment on the face
- Care needed to prevent entry of preparation into the eye
- Hazardous use in psoriasis
- Use of appropriate anti-microbial therapy for infected lesions
Use in Special Populations
- Avoid long-term continuous therapy in children under 12 years of age
Overdose Effects
- Chronic over-dosage may lead to hypercortisolism
