Indications

  • Acute bacterial exacerbation of chronic bronchitis in adults
  • Acute maxillary sinusitis
  • Community-acquired pneumonia
  • Pharyngitis or tonsillitis
  • Uncomplicated skin and skin structure infections
  • Acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibition of microsomal protein synthesis
  • Binding to the donor site on the 50S subunit of the bacterial ribosome
  • Prevention of translocation to the donor site
  • Activity against most Gram-positive bacteria and Chlamydia
  • Activity against some Gram-negative bacteria and Mycoplasmas
  • Twice the activity of Erythromycin against H. influenzae

  • Dosage for adults
  • Combination dosing regimens for H. pylori infection
  • Dosage for children
  • Dosage in renal and hepatic impairment

Interaction

  • Contraindicated co-administration with HMG-CoA reductase inhibitors
  • Contraindicated co-administration with gastroprokinetic agents
  • Contraindicated co-administration with ergot alkaloids
  • Contraindicated co-administration with antipsychotics
  • Contraindicated co-administration with anti-gout agents
  • Not recommended co-administration with antiarrhythmics drugs

Contraindications

  • Known hypersensitivity to clarithromycin
  • Cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

  • Discontinue if signs and symptoms of hepatitis occur
  • Increased risk of all-cause mortality after treatment
  • Evaluate for Clostridium difficile associated diarrhea if diarrhea occurs

Use in Special Populations

  • Safety and effectiveness in pediatric patients 6 months and older
  • Safety and effectiveness in prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients 20 months and older with advanced HIV infection

Overdose Effects

  • Ingestion of large amounts can produce gastro-intestinal symptoms
  • Prompt elimination of unabsorbed drug and supportive measures for adverse reactions
