Indications
- Reduction of total cholesterol levels

- Prevention of cardiac ischaemic events
- Treatment of hypercholesterolemia associated with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors
- Rapid absorption after oral administration
- High plasma protein binding
- Extensive metabolism to active metabolites
- Primary route of elimination is through bile
- Plasma elimination half-life is approximately 14 hours
- Systemic availability of HMG-CoA reductase inhibitory activity is approximately 30%
Dosage & Administration
- Initial adult dosage: 10 mg once daily
- Adult dosage range: 10 to 80 mg once daily
- Initial child (10-18 years) dosage: 10 mg once daily
- Child dosage range: 10 to 20 mg once daily
- Titration intervals of at least 4 weeks
- Dosage adjustment based on lipid levels analysis
- Can be administered with or without food
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Plasma concentration decrease with coadministration of antacid
- Plasma concentration decrease with coadministration of colestipol
- Steady-state plasma digoxin concentrations increase with coadministration of digoxin
- Plasma concentration increase with coadministration of erythromycin
- AUC values increase with coadministration of oral contraceptives
- No clinically significant effect on prothrombin time with warfarin coadministration
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease
- Unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to HMG-CoA reductase inhibitors

- Caution in patients consuming substantial quantities of alcohol
- Caution in patients with a history of liver disease
- Discontinue therapy in case of markedly elevated CPK levels or myopathy
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects
- No clinically significant difference between men and women
- No influence of renal disease on plasma concentrations or LDL-C reduction
- Markedly increased plasma concentrations in patients with chronic alcoholic liver disease
Overdose Effects
- No specific treatment available
- Symptomatic treatment and supportive measures
- Liver function tests and serum CK levels monitoring
- Hemodialysis not expected to significantly enhance clearance
