Pharmacology
- Proton pump inhibitor
- Suppression of gastric acid secretion
- Inhibition of H+/K+-ATPase in gastric parietal cell
- Increased acid control compared to racemic proton pump inhibitors
- Enteric-coated pellet formulation of esomeprazole magnesium
- Absorption within approximately 1.5 hours after oral administration
- Approximately 90% systemic bioavailability with once daily dosing
- Extensive liver metabolism by CYP enzyme system
- 97% plasma protein binding
- Plasma elimination half-life of approximately 1–1.5 hours
- Less than 1% of excretion of parent drug in urine
- Combination therapy with clarithromycin and amoxicillin
- Increase in AUC and Cmax during triple combination therapy
Dosage
- 20 mg or 40 mg Once Daily for 4-8 Weeks for erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD
- Triple Therapy for Helicobacter Pylori eradication
- 20-80 mg Once Daily for Zollinger-Ellison Syndrome
- 20-40 mg Once Daily for acid-related Dyspepsia
- 20 mg Once Daily for 2-4 weeks for Duodenal ulcer
- 20-40 mg Once Daily for 4-8 weeks for Gastric ulcer
- IV Injection or IV infusion dosing for specific conditions
Administration
- Oral administration one hour before meals
- Preparation of oral suspension
- IV injection after reconstitution with 0.9% Sodium Chloride
- Specific administration instructions for nasogastric or gastric tube
Interaction
- Extensive metabolism in the liver by CYP2C19 and CYP3A4
- Inhibition of gastric acid secretion affecting drug absorption
- No clinically significant interactions with specific drugs
Contraindications
- Known hypersensitivity to any of the formulation

- No overall differences in safety and efficacy observed in elderly
- No dosage adjustment recommended for mild to moderate hepatic insufficiency
- Expected pharmacokinetics in patients with renal impairment
- No alteration in pharmacokinetics relative to healthy volunteers
Overdose Effects
- Lethal effects observed in rats at high doses
- Reduced motor activity, respiratory frequency changes, tremor, ataxia, and convulsions

- Symptomatic and supportive treatment recommended
