Pharmacology
- Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1)
- Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are products of arachidonic acid metabolism released from various cells, including mast cells and eosinophils
- Correlated with the pathophysiology of asthma & allergic rhinitis, including airway edema and smooth muscle contraction
Dosage & Administration
- Adults and adolescents dosage: Montelukast 10 mg tablet once daily
- Pediatric patients dosage: 6 to 14 years - Montelukast 5 mg, 2 to 5 years - Montelukast 4 mg, 6 months to 5 years - Montelukast 4 mg oral granules
Interaction
- No clinically important effects on the pharmacokinetics of drugs like theophylline, prednisone, terfenadine, digoxin, and warfarin
- Used concomitantly with thyroid hormones, sedative hypnotics, non-steroidal anti-inflammatory agents, benzodiazepines, and decongestants without evidence of adverse interactions
Contraindications
- Hypersensitivity to any component of this product
Side Effects
- Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, haemorrhage, irritability, malaise, muscle complaints, oedema
- Rare: Angioedema, concentration impaired, disorientation, eosinophilic granulomatosis with polyangiitis, erythema nodosum, hallucination, hepatic disorders, memory loss, palpitations, pulmonary eosinophilia, suicidal tendencies, tremor
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Caution should be exercised when given to a nursing mother
Precautions & Warnings
- Not indicated for use in the reversal of bronchospasm in acute asthma attacks
- Should not be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment and management of exercise-induced bronchospasm
Overdose Effects
- Most frequently occurring adverse experiences: abdominal pain, somnolence, thirst, headache, vomiting, psychomotor hyperactivity
- Usual supportive measures should be employed in the event of overdose
