Indications

Pharmacology

  • Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1)
  • Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are products of arachidonic acid metabolism released from various cells, including mast cells and eosinophils
  • Correlated with the pathophysiology of asthma & allergic rhinitis, including airway edema and smooth muscle contraction

Dosage & Administration

  • Adults and adolescents dosage: Montelukast 10 mg tablet once daily
  • Pediatric patients dosage: 6 to 14 years - Montelukast 5 mg, 2 to 5 years - Montelukast 4 mg, 6 months to 5 years - Montelukast 4 mg oral granules

Interaction

  • No clinically important effects on the pharmacokinetics of drugs like theophylline, prednisone, terfenadine, digoxin, and warfarin
  • Used concomitantly with thyroid hormones, sedative hypnotics, non-steroidal anti-inflammatory agents, benzodiazepines, and decongestants without evidence of adverse interactions

Contraindications

  • Hypersensitivity to any component of this product

Side Effects

  • Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
  • Uncommon: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, haemorrhage, irritability, malaise, muscle complaints, oedema
  • Rare: Angioedema, concentration impaired, disorientation, eosinophilic granulomatosis with polyangiitis, erythema nodosum, hallucination, hepatic disorders, memory loss, palpitations, pulmonary eosinophilia, suicidal tendencies, tremor

Pregnancy & Lactation

  • Crosses the placenta in rats and rabbits
  • Caution should be exercised when given to a nursing mother

Precautions & Warnings

  • Not indicated for use in the reversal of bronchospasm in acute asthma attacks
  • Should not be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment and management of exercise-induced bronchospasm

Overdose Effects

  • Most frequently occurring adverse experiences: abdominal pain, somnolence, thirst, headache, vomiting, psychomotor hyperactivity
  • Usual supportive measures should be employed in the event of overdose
