Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
- To reduce the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease
Pharmacology
- Sodium glucose co-transporter-2 (SGLT-2) inhibitor
- Reduces renal absorption of glucose
- Increases glucose excretion
- Assists weight loss
- Lowers blood pressure
Dosage & Administration
- Recommended dose: 10 mg once daily
- May be increased to 25 mg once daily in patients tolerating the medication
- Can be taken with or without food
- In cases of volume depletion, correction is recommended prior to initiation
Interactions
- Co-administration with diuretics increases urine volume
- Co-administration with insulin or insulin secretagogues increases the risk for hypoglycemia
- Monitoring glycemic control with urine glucose tests not recommended
- Monitoring glycemic control with 1,5-anhydroglucitol assay not recommended
Contraindications
- History of serious hypersensitivity reaction to Empagliflozin or any of its ingredients
- Severe renal impairment
- End-stage renal disease
- Dialysis

- Female genital mycotic infections
- Dehydration
- Hypotension
- Weakness
- Dizziness
- Increased thirstiness
Pregnancy & Lactation
- Limited studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Not recommended while breastfeeding
Precautions & Warnings
- Assessment of renal function recommended prior to initiation
- Should not be initiated in patients with eGFR less than 45 ml/min/1.73 m^2
- No dose adjustment needed in patients with eGFR greater than or equal to 45 ml/min/1.73 m^2
Overdose Effects
- Usual supportive measures recommended in case of overdose
- Hemodialysis effectiveness not studied
