Indications
- Treatment of chronic angina
- Decrease angina episodes in patients with coronary artery disease
- Use with beta-blockers, nitrates, calcium channel blockers, anti-platelet therapy, lipid-lowering therapy, ACE inhibitors, and angiotensin receptor blockers
Pharmacology
- Anti-ischemic effects
- Antianginal effects
- Inhibition of cardiac late sodium current (I_Na)
- Inhibition of I_Kr
- Absorption: C_max at 2-5 h
- Half-life: 6-22 h
- Distribution: Approximately 62% bound to human plasma proteins
- Metabolism by CYP3A and CYP2D6
- Excretion mainly in urine and feces
Dosage & Administration
- Initiate dosing at 500 mg twice daily
- Increase to 1000 mg twice daily if needed
- Swallow tablets whole, do not crush, break or chew
- Maximum recommended daily dose is 1000 mg twice daily
- Prescribed dose may be taken at the next scheduled time if missed, without doubling the next dose
Interaction
- Avoid use with strong CYP 3A inhibitors
- Limit maximum dose with moderate CYP 3A inhibitors
- Consider dose adjustment with P-gp inhibitors or CYP2D6-metabolized drugs
Contraindications
- Pre-existing QT prolongation
- Hepatic impairment
- Taking QT prolonging drugs
- Taking potent and moderately potent CYP3A inhibitors

- Abdominal pain
- Dry mouth
- Vomiting
- Peripheral edema
- Dyspnea
- Hypotension
- Orthostatic hypotension
Pregnancy & Lactation
- Pregnancy Category C
- No adequate studies on its effect on the developing fetus
- No well-controlled studies in pregnant women
- Caution in nursing mothers
- Potential risk to the fetus
Precautions & Warnings
- Blocks QTc and prolongs the QTc interval
- Caution when co-administered with digoxin or P-gp inhibitors
- No increased risk of proarrhythmia or sudden death in acute coronary syndrome population
Overdose Effects
- Dose-related increases in dizziness, nausea, and vomiting
- Diplopia
- Paresthesia
- Confusion
- Syncope
- General supportive measures needed
- Continuous ECG monitoring warranted
- Hemodialysis unlikely to be effective
