Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients
Pharmacology
- Lowering both basal and postprandial plasma glucose
- Decreasing hepatic glucose production
- Decreasing intestinal absorption of glucose
- Improving insulin sensitivity
Dosage & Administration
- Usual starting dose of Metformin is 500 mg twice a day or 850 mg once a day for adults
- Dosage increases should be made in increments of 500 mg weekly or 850 mg every 2 weeks, up to a total of 2000 mg per day, given in divided doses
- Usual starting dose of Metformin in children is 500 mg twice a day
- Swallow Metformin XR tablet whole and never crush, cut, or chew
Interaction
- Interaction with Furosemide is not available
- Nifedipine appears to enhance the absorption of Bigmet XR
- No effect on Cimetidine pharmacokinetics
- Potential interaction with cationic drugs eliminated by renal tubular secretion
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Conditions that may alter renal function
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Taste disturbance
- Gastrointestinal disorders
- Liver function test abnormalities or hepatitis
- Skin reactions
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy is associated with increased risk of congenital abnormalities and perinatal mortality
- Metformin is excreted into human breast milk
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantial excretion by the kidney
- Lowering vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Caution in elderly subjects
- No effect on growth and puberty detected during controlled clinical studies
- Regular assessment of renal function necessary in elderly subjects
Use in Special Populations
- Dosage adjustment in elderly subjects based on renal function
- Careful follow-up of the effect on growth and puberty in metformin-treated children recommended
- Particular caution recommended when prescribing to children aged between 10 and 12 years
- Creatinine clearance determination before initiating treatment and regularly thereafter
Overdose Effects
- Hypoglycemia not seen with Bigmet XR doses up to 85 gm
- Lactic acidosis is a medical emergency and must be treated in the hospital
- Hemodialysis for removing lactate and Bigmet XR in lactic acidosis
