Indications
- Treatment of adults with type 2 diabetes mellitus

Pharmacology
- Empagliflozin is an SGLT2 inhibitor
- Metformin Hydrochloride is a biguanide type oral antihyperglycemic drug
- Empagliflozin reduces renal reabsorption of filtered glucose
- Metformin lowers basal and postprandial plasma glucose
Dosage & Administration
- Individualized based on effectiveness and tolerability
- Take twice daily with meals
- Dose escalation should be gradual
- Maximum recommended daily doses
Interaction
- Diuretics increase urine volume and frequency of voids
- Co-administration with insulin increases risk for hypoglycemia
- Monitoring glycemic control with urine glucose tests is not recommended
- Drugs that reduce Metformin clearance may increase accumulation
Contraindications
- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis
- Diabetic pre-coma
- Severe renal failure
- Acute conditions altering renal function
- Diseases causing tissue hypoxia
- Hepatic impairment
- Acute alcohol intoxication
- Alcoholism

- Lactic acidosis

- Urosepsis, Pyelonephritis, Fournier’s gangrene & genital mycotic infections
- Hypoglycemia
- Vitamin B12 Deficiency
- Increased LDL-C
- Macrovascular Outcomes
Overdose Effects
- No toxicity with single doses of up to 800 mg Empagliflozin
- Hypoglycemia not seen with Metformin doses of up to 85 g
- Lactic acidosis is a medical emergency
- Treatment should be initiated as appropriate to the patient's clinical status
