Description

Indications
- Treatment of metastatic non-small cell lung cancer (NSCLC) with ALK-positive or ROS1-positive tumors
Dosage & Administration
- Recommended dose: 250 mg orally twice daily
- Geriatric use: No observed differences in safety or efficacy between older and younger patients
- Pediatric dose: Safety and effectiveness not established
- Renal impairment: Dosage adjustment required for severe renal impairment
- Hepatic impairment: Caution advised in patients with hepatic impairment
Mode of Action
- Inhibits receptor tyrosine kinases including ALK, ROS1, and c-Met
- Demonstrated concentration-dependent inhibition of ALK, ROS1, and c-Met phosphorylation in cell-based assays
- Predominantly metabolized by CYP3A4/5
- Mean apparent plasma terminal half-life: 42 hours
Interaction
- Avoid concurrent use with strong CYP3A inhibitors and inducers
- Avoid with CYP3A substrates with narrow therapeutic indices
Side Effects
- Common adverse reactions include vision disorders, nausea, diarrhea, vomiting, edema, constipation, elevated transaminases, fatigue, decreased appetite, upper respiratory infection, dizziness, and neuropathy
Pregnancy & Lactation
- Can cause fetal harm when administered to a pregnant woman
- No available data on its use during pregnancy
- Patients advised to use effective contraception
- Not recommended during breastfeeding and for 45 days after final dose
Precautions & Warnings
- Periodic liver testing advised due to hepatotoxicity risk
- Permanent discontinuation in case of ILD/Pneumonitis
- Monitoring of QT interval prolongation and bradycardia
- Ophthalmological evaluation recommended for severe visual loss
