Indications

  • Treatment of chronic hepatitis B virus infection
  • For adults with compensated liver disease

Pharmacology

  • Tenofovir alafenamide as a nucleoside analog reverse transcriptase inhibitor
  • Converted to tenofovir through hydrolysis by carboxylesterase 1 (CES1)
  • Inhibits HBV replication through incorporation into viral DNA
  • Weak inhibitor of mammalian DNA polymerases

Dosage & Administration

  • Recommended dosage of 25 mg once daily with food
  • No dosage adjustment required in patients with certain renal impairment
  • Not recommended in patients with decompensated hepatic impairment
  • Safety and effectiveness not established in pediatric patients
  • Administered to elderly subjects with no clinically significant differences observed

Interaction

  • Substrate of P-glycoprotein (P-gp) and BCRP
  • Drugs affecting P-gp activity may lead to changes in absorption
  • Coadministration with other drugs may affect absorption and plasma concentration
  • Drugs affecting renal function may increase concentrations

Pregnancy & Lactation

  • Prevalence of birth defects in live births following exposure to TAF-containing regimens

Precautions & Warnings

  • Risk of severe acute exacerbations of hepatitis B after discontinuation of treatment
  • Risk of development of HIV-1 resistance in patients coinfected with HBV and HIV-1
  • Renal impairment including acute renal failure reported
  • Lactic acidosis and severe hepatomegaly with steatosis reported

Overdose Effects

  • Monitor patient for evidence of toxicity
  • General supportive measures for overdose
  • Efficiently removed by hemodialysis
