Indications

  • Symptomatic treatment of chronic constipation in adults

Pharmacology

  • Acts as a selective stimulator of 5-HT4 receptors
  • Produces release of acetylcholine in the GI tract
  • Stimulates motility by interacting with 5-HT4 receptors

Dosage & Administration

  • Adult dosage: 2 mg once daily with or without food
  • Older people dosage: Start with 1 mg once daily, may be increased to 2 mg once daily if needed
  • Not recommended for children and adolescents younger than 18 years

Interaction

  • Low interaction potential with CYP-mediated metabolism of co-medicated medicinal products
  • May have interactions with potent inhibitors of P-gp such as ketoconazole, verapamil, cyclosporine A, and quinidine
  • No clinically relevant effects on the pharmacokinetics of warfarin, digoxin, alcohol, paroxetine, or oral contraceptives

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Renal impairment requiring dialysis

Side Effects

  • Most frequently reported adverse reactions: headache, abdominal pain, nausea, diarrhea
  • Adverse reactions occur predominantly at the start of therapy and usually disappear within a few days
  • Majority of adverse events were mild to moderate in intensity

Pregnancy & Lactation

  • Not recommended during pregnancy
  • Women of childbearing potential should use effective contraception
  • Not recommended during breastfeeding based on animal studies

Precautions & Warnings

  • A dose of 1 mg recommended in subjects with severe renal impairment
  • Caution in prescribing to patients with severe hepatic impairment (Child-Pugh class C)
  • Possible reduction in efficacy of oral contraceptives in case of severe diarrhea

Use in Special Populations

  • Dose for patients with severe renal impairment: 1 mg once daily
  • No dose adjustment required for patients with mild to moderate renal impairment
  • Patients with severe hepatic impairment start with 1 mg once daily, may be increased to 2 mg if required and well tolerated
  • No dose adjustment required for patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms may include headache, nausea, and diarrhea
  • Specific treatment not available for overdose
  • Extensive fluid loss by diarrhea or vomiting may require correction of electrolyte disturbances
