Description
- A HMG-CoA reductase inhibitor used to treat high cholesterol and certain lipid disorders
- Selectively inhibits hepatic HMG-CoA reductase, reducing cholesterol synthesis
- Helps lower LDL-C, total-C, ApoB, non HDL-C, and TG levels and increase HDL-C
- Indicated for heterozygous familial hypercholesterolemia, hypertriglyceridemia, dysbetalipoproteinemia, and prevention of MI and stroke
- Administration ranges from 5mg to 40mg depending on condition and patient group
Dosage
- Usual dose range: 5-20 mg/day; max 20 mg/day for pediatric patients
- 40 mg once daily for very high cholesterol and high risk of MI or stroke
- Initiation with 5 mg once daily for Asian patients
- Avoid using with certain medications to limit exposure
Pharmacology
- Competitive inhibitor of HMG-CoA reductase, reducing cholesterol synthesis
- Increases hepatic LDL receptors and inhibits synthesis of VLDL
Interaction
- Possible interactions with Cyclosporine, Gemfibrosil, Lopinavir/Ritonavir, Atazanavir/Ritonavir, and coumarin anticoagulants
Contraindications
- Contraindicated in patients with known hypersensitivity, liver disease, pregnancy, and nursing mothers
Side Effects
- Well-tolerated with infrequent adverse events like headache, myalgia, constipation, and nausea
Pregnancy & Lactation
- Administered to women of childbearing age only under minimal pregnancy risk
Precautions & Warnings
- Caution in patients with predisposing factors for myopathy, renal impairment, and concurrent lipid-lowering therapies
- Periodic liver enzyme tests recommended
- Therapy should be discontinued if markedly elevated creatinine kinase levels occur
