Indications
- Prevention of postoperative nausea and vomiting (PONV)
- Prevention of Chemotherapy Induced Nausea and Vomiting (CINV)
Pharmacology
Aprepitant is a selective high affinity antagonist of human substance P neurokinin 1 (NK1) receptors. Aprepitant stops substance P from binding to the NK1 receptors, preventing nausea and vomiting.
Dosage
- Post Operative Nausea and Vomiting: 40 mg oral dosage within 3 hours prior to anesthesia induction
- Chemotherapy-Induced Nausea and Vomiting:
- Day 1: Aprepitant 125 mg orally, Dexamethasone 12 mg orally, 5-HT3 antagonist (Ondansetron) 24 mg 30 minutes before chemotherapy
- Day 2: Aprepitant 80 mg orally, Dexamethasone 8 mg orally
- Day 3: Aprepitant 80 mg orally, Dexamethasone 8 mg orally
- Day 4: Dexamethasone 8 mg orally
Administration
Aprepitant may be taken with or without food. No dosage adjustment is necessary for elderly patients.
Interaction
Vomend is a substrate, an inhibitor, and an inducer of CYP3A4. Precautions should be taken while coadministering with drugs utilizing CYP3A4 or CYP2C9. Efficacy of hormonal contraceptives may be reduced.
Contraindications
Aprepitant is contraindicated in hypersensitive patients and should not be used concurrently with specific drugs.

Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy if clearly needed. Discontinue nursing or discontinue the drug based on patient's importance.
Use in Special Populations
- Patients with Renal Impairment: No dosage adjustment necessary for patients with renal impairment or end-stage renal disease (ESRD) undergoing hemodialysis.
- Patients with Hepatic Impairment: No dosage adjustment necessary for patients with mild to moderate hepatic impairment. No clinical data for severe hepatic impairment.
