Indications
- Treatment of hypertension
- Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption
Dosage
- Initial dose of 50/12.5 is one tablet once daily
- Antihypertensive effect is attained within three weeks after initiation of therapy
- No initial dosage adjustment is necessary for elderly patients
Administration
- May be administered with or without food
- Can be administered with other antihypertensive agents
Interaction
- Potential interactions with potassium-sparing diuretics
- Dosage adjustment of antidiabetic drugs may be required
- Additive effect or potentiation with other antihypertensive drugs
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
- Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs
Side Effects
- Symptomatic hypotension including dizziness may occur
- Hyperkalemia occurs occasionally
- Angioedema and vertigo reported as side-effects
Pregnancy & Lactation
- Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
- Not recommended in breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes should be performed at appropriate intervals
- Hypokalemia may rarely develop
Use in Special Populations
- Therapy with 50/12.5 may be followed as long as creatinine clearance is >30 ml/min
- Loop diuretics are preferred in patients with severe renal impairment
- Not recommended in pediatric patients
Overdose Effects
- Manifestation of overdosage could be hypotension and tachycardia
- Supportive treatment should be instituted in case of symptomatic hypotension
