Indications

  • Treatment of hypertension
  • Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Losartan and its metabolite block the vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily
  • Antihypertensive effect attained within three weeks after initiation of therapy

Administration

  • Can be administered with or without food
  • May be administered with other antihypertensive agents

Interaction

  • No significant drug-drug pharmacokinetic interactions observed with specific drugs
  • Dosage adjustment required for antidiabetic drugs when administered concurrently with Hydrochlorothiazide

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria or hypersensitivity to other sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reported angioedema, vertigo, gastrointestinal disturbances, etc.

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • May rarely develop hypokalemia
  • Monitoring for impaired renal function and symptomatic hypotension

Use in Special Populations

  • Use in Patients with Renal Impairment
  • Not recommended in patients with more severe hepatic impairment
  • Safety and effectiveness not established in pediatric patients

Overdose Effects

  • Manifestations of overdosage such as hypotension and tachycardia
  • Supportive treatment for symptomatic hypotension in case of Losartan overdosage
