Indications

  • Treatment of hypertension
  • Reduction of risk of stroke in hypertensive patients with left ventricular hypertrophy

Pharmacology

  • Losartan and its active metabolite block vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Antihypertensive effect is attained within three weeks after initiation of therapy
  • No initial dosage adjustment necessary for elderly patients

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug pharmacokinetic interactions found in interaction studies
  • Concomitant use of potassium-sparing diuretics or salt substitutes containing potassium may lead to increase in serum potassium
  • When administered concurrently, alcohol, barbiturates, or narcotics may potentiate orthostatic hypotension

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria
  • Hypersensitivity to other sulfonamide-derived drugs

  • Hypokalemia may rarely develop
  • Impaired renal function
  • Symptomatic hypotension

Use in Special Populations

  • In patients with renal impairment, loop diuretics are preferred to thiazides
  • Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Manifestations of overdosage may include hypotension, tachycardia, dehydration, and hypokalemia
