Indications
- Treatment of adult patients with primary biliary cholangitis (PBC)
- Combination therapy with ursodeoxycholic acid (UDCA)
Mechanism of Action
- Agonist for Farnesoid X Receptor (FXR)
- Regulator of bile acid, inflammatory, fibrotic and metabolic pathways
Pharmacodynamics
- Increased concentrations of FGF-19
- Reduction in concentrations of cholic acid and chenodeoxycholic acid
Cardiac Electrophysiology
Does not prolong the QT interval to any clinically relevant extent
Dosage & Administration
- Start with a dosage of 5 mg once daily for the first 3 months
- Monitoring to assess safety
- Management of patients with intolerable pruritus

- Bile Acid Binding Resins
- Warfarin
- CYP1A2 Substrates with Narrow Therapeutic Index
- Inhibitors of Bile Salt Efflux Pump
Contraindications
- Decompensated cirrhosis
- Compensated cirrhosis with evidence of portal hypertension
- Complete biliary obstruction
Side Effects
- Common: Pruritus, Fatigue, Stomach pain and discomfort, Rash, Arthralgia, Oropharyngeal pain
- Other common: Dizziness, Constipation, Abnormal thyroid function, Eczema
Pregnancy & Lactation
- Limited available human data on the use during pregnancy
- No information on the presence in human milk
Precautions & Warnings
- Hepatic Decompensation and Failure in PBC Patients with Cirrhosis
- Severe Pruritus
- Reduction in HDL-C
